Power of Attorney - Documents

When uploading a Power of Attorney (POA) to manage care for a loved one on Carelancer, additional documentation is required for verification purposes.

Required Documents for Verification:

  1. Photo Identification (ID)

    • Provide a government-issued photo ID for both:
      • The coordinator (agent) managing the care.
      • The loved one (subject) receiving care.
    • This ensures that their identities match the information listed on the Power of Attorney.

  2. Notarized Power of Attorney Document

    • Upload the full, notarized Power of Attorney document to confirm legal authority.

Review Process:

  • The Carelancer support team will review all submitted documents.
  • Profiles that meet the verification requirements will be approved accordingly.
  • If additional information is needed, support will reach out via email.
