Power of Attorney - Documents
When uploading a Power of Attorney (POA) to manage care for a loved one on Carelancer, additional documentation is required for verification purposes.
Required Documents for Verification:
Photo Identification (ID)
- Provide a government-issued photo ID for both:
- The coordinator (agent) managing the care.
- The loved one (subject) receiving care.
- This ensures that their identities match the information listed on the Power of Attorney.
Notarized Power of Attorney Document
- Upload the full, notarized Power of Attorney document to confirm legal authority.
Review Process:
- The Carelancer support team will review all submitted documents.
- Profiles that meet the verification requirements will be approved accordingly.
- If additional information is needed, support will reach out via email.
